Working with Fidelis

Working together, we can make a real difference

Much has been written about the "graying of America." The U.S. population age 65 and over is expected to double in size within the next 25 years. By 2030, almost 1-out-of-5 Americans — some 72 million people — will be 65 years or older. The age group 85 and older is now the fastest growing segment of the U.S. population, projected to double to 9.3 million in the same time period.¹

Mostly due to medical advances plus a growing emphasis on preventive care, many studies have forecast a decline in the disability rate among our aging population and, consequently, an increase in the life expectancy rate. Given the coming explosion in the senior population, however, a decline in disability rate does not translate into a decline in the number of people who will need healthcare services.

Along with increased life expectancy and decreased disability has come a shift in the causes of death and a rise of chronic disease. At least 80 percent of older Americans are living with one chronic disease, 50 percent have at least two. Over two thirds of current healthcare costs are for treating chronic illnesses; among older Americans, almost 95 percent of healthcare expenditure is for chronic diseases. ²

So, even as progress is made in improving life expectancy, longevity is often accompanied by greater prevalence of chronic conditions, multiple co-morbidities, pain and disability. Fidelis SecureCare plans have been specifically developed to meet the growing challenge. In each of our service areas we contract with highly qualified medical professionals who have extensive training in the care and management of seniors living in nursing homes and assisted living communities. We then apply our unique business model, designed to

  • maximize hands-on, personal care of each Member resident to improve clinical outcomes
  • increase cost savings for Members and long term care residences
  • increase facility revenue via premiums on Medicare rates and administrative fees
  • incent residence performance via quality bonuses

We have shown that the quality of life of our Members can be significantly improved through the comprehensive management of their medical care:

  • Increasing primary care services in the Member’s residence
  • Providing early treatment of disease occurrences or complications in the location most conducive to treatment
  • Reducing unnecessary hospitalizations and readmissions
  • Investing in psychological support services and activities for patients and families
  • Basing care managers on-site to improve care coordination and communication

Most important, our fully integrated, biopsychosocial approach to care creates the opportunity for our Members to spend their final years with dignity and an improved quality of life.

We welcome inquiries from nursing home and assisted living community owner/operators and physician practices interested in improving the quality of care—and therefore the quality of life—of their patients and residents.
