The Fidelis Formulary provides up-to-date information about all of the drugs covered by your Fidelis SecureCare health plan. The link below accesses our Drug Search tool, a user-friendly electronic library you can use to look up your prescription medicine and find out

• The tier assigned to that drug
• Quantity limitations, if any
• Lower cost alternatives, if available
• The necessary forms for requesting an exception to coverage limitations and for Prior Authorization (PA) of a drug
