The Fidelis Formulary provides up-to-date information about all of the drugs covered by your Fidelis SecureCare health plan. The link below accesses our Drug Search tool, a user-friendly electronic library you can use to look up your prescription medicine and find out
- The tier assigned to that drug
- Quantity limitations, if any
- Lower cost alternatives, if available
- The necessary forms for requesting an exception to coverage limitations and for Prior Authorization (PA) of a drug

If your drug is not included in this formulary, you should contact Member Services at (877) 372-8085. Our hours of operation are 8:00 a.m. to 6:00 p.m., Monday through Friday. TTY Users, call (888) 844-5530.
If you’re new to search tools, refer to the instructions below. Note: the red arrows are for guidance and will not exist on the actual page.
Just choose your plan, then click on "Look up drugs"

Type in your prescription drug and select the correct dosage from the drop down menu that will appear after you start typing.

The next screen will display the details for your selected drug. If you want to search another drug, return to the “Add a drug” box and type it in. After you choose the drug from the drop down list it will be added immediately above your first drug entry.
Note: if there are lower cost alternatives available to you, an orange button will appear under the drug name and dose line. Just click for information.

If you need a Prior Authorization form or a Request for Exception to Quantity Limitation form, click on the "Forms & PAs" button at the top.

Once you've clicked the "Forms & PAs" button, buttons for downloading the forms in PDF format will appear in the lower right corner of your screen. Click on the button of the form you wish to download.

When you select a form, a dialog box will present you with two options:

To view the form in the browser choose "Open with Adobe Acrobat". You can still choose to save it from the browser’s file menu after you've opened it simply by going to "file" > "save as".

To download the form to your computer choose "Save to Disk"
