Fidelis offers three specially designed plans that provide total care for all your medical needs, and include the following benefits:
- Comprehensive Part D Prescription Drug coverage with copays as low as $5.00 for some plans
- Ongoing prescription monitoring and management
- Scheduling of, and transportation to, necessary medical appointments
- Specially designed care plan customized to each Member’s medical requirements
- A dedicated Fidelis Care Manager
- No coinsurance or copays for in-network doctor visits
- Low copay for emergency or urgent care
- Preventive services, such as: immunizations, colorectal screening, bone mass measurement, mammography, prostate and breast exams
- Annual dental exam and denture coverage
- Health and wellness programs
- Routine and diagnostic hearing care with hearing aid coverage
- Vision care, including eyeglasses or contacts
