Benefits that go beyond those traditional Medicare can offer |
Seniors living in nursing home and assisted living communities now have more healthcare options. Our Medicare Advantage Special Needs Plans offer comprehensive healthcare benefits — including Part D prescription drug coverage — to eligible individuals.
Fidelis SecureCare expands coverage beyond original Medicare with benefits vital to Member health, plus no deductibles* and low or no co-pays for services, such as:
- On-site primary care team visits
- Complete Part D Prescription Drug coverage
- Assessment and hands-on support of each Member’s care treatment plan
- Monitoring and management of prescribed medications
- Dedicated Fidelis Care Manager to coordinate all providers involved in each Member’s ongoing care
- Medically necessary transportation to the hospital or physician’s office
- Preventive care, screenings, vision/dental/hearing and podiatry services
- Emergency room, ambulance and urgent care
- Skilled nursing services during a covered Medicare stay at a skilled nursing facility
- Waiver of Medicare’s three-day hospital stay requirement to qualify for skilled care

Eligibility
You are eligible for Fidelis SecureCare if you...
- Are covered by Medicare Parts A and B
- Have not been diagnosed with end-stage renal disease
- Have been a resident in a nursing home or an assisted living community (or anticipate you will be a resident) for at least 90 days
- Meet certain clinical criteria necessary to enroll in Medicare Advantage Special Needs Plans
- Live in our service area
