Your Evidence of Coverage is an important legal document that will be helpful to you as a Member. It provides you with details about your Fidelis SecureCare plan, including
- Your Plan’s service area
- How to access Part D benefits, including how to fill a prescription through retail pharmacy and mail order service
- Your Plan’s benefits, including annual deductible, amount of initial coverage limit and cost-sharing
